HIGH SCHOOL TRANSCRIPT REQUESTS

Students from the Pulaski County School System can request transcripts, which are in the form of permanent record cards and/or from teachers’ registers, at the district’s central office (501 University Drive, P. O. Box 1055, Somerset, KY 42502-1055). Permanent record cards and some teachers’ registers are on microfilm. Identification is required and a request form must be submitted.

Former students who need proof of age can request this from the system’s central office. Identification is required and a request form must be submitted.

HISTORICAL/DESCENDANT/GENEALOGY REQUESTS

Request for copies of employee/student records from the Pulaski County School District for historical/descendant/genealogy purposes can be obtained at the system’s central office, records management. Identification is required and a request form must be completed stating the reason for the request.

ARCHIVES INFORMATION REQUESTS

Request for Pulaski County Schools archives information can be obtained at the system’s central office, records management. Identification is required as well as a written reason for information. A request form must be completed.

ATTENDEES AT ONE- AND/OR TWO-ROOM SCHOOLS

Request and reason for a list of attendees at one of the old one- and/or two- room schools prior to 1964 can be made with records management at the Pulaski County School System’s Central Office. Identification is required and a request form must be completed.
